You will need to have with you for your first visit:
- Claim Number
- Date of Injury/Accident, insurance report of injury/accident
- Attending Physician or Chiropractor, name, phone number, office address, Rx/prescription from doctor for massage therapy with Dx/diagnosis codes.
- Insurance claim representatives name and claims mailing address
